Hospice Social Work in Chicago

What Hospice Social Workers Do
Hospice social workers are trained professionals who address the emotional, practical, and social needs of patients and families facing terminal illness. Unlike other members of the hospice team who focus primarily on physical care, social workers concentrate on the well-being of the whole person and the entire family system.
Our Chicago social workers serve as advocates, counselors, problem-solvers, and guides. They help families understand what’s happening, access needed resources, make important decisions, and cope with the emotional challenges of end-of-life care.
Services Our Social Workers Provide
Emotional Support and Counseling
Facing death brings up difficult emotions for everyone involved. Our social workers provide:
- Individual counseling for patients dealing with fear, anger, sadness, or acceptance
- Family counseling to help loved ones process their emotions together
- Support for children who need age-appropriate help understanding what’s happening
- Pre-bereavement counseling to help families prepare for loss
- A safe, non-judgmental space to express feelings
Chicago families appreciate having someone who simply listens without trying to fix everything, someone who understands grief and knows how to hold space for difficult emotions.
Advance Care Planning
Making decisions about end-of-life care is never easy. Our social workers help with:
- Completing advance directives and living wills
- Understanding healthcare power of attorney
- Discussing do-not-resuscitate (DNR) orders
- Clarifying goals of care and patient wishes
- Facilitating family conversations about difficult decisions
These documents ensure your loved one’s wishes are respected and honored when they can no longer speak for themselves.
Family Communication and Conflict Resolution
Terminal illness can strain even the strongest families. Disagreements about treatment, old conflicts resurfacing, and the stress of caregiving can create tension. Our social workers help by:
- Facilitating family meetings where everyone’s voice is heard
- Mediating conflicts between family members
- Helping families set realistic expectations
- Addressing unfinished business or unresolved issues
- Fostering understanding and cooperation among family members
Practical Assistance and Resource Connection
Beyond emotional support, our social workers provide practical help:
| Type of Assistance | What We Provide |
|---|---|
| Financial Resources | Information about Medicare, Medicaid, veterans benefits, financial assistance programs |
| Insurance Navigation | Help understanding coverage and completing paperwork |
| Community Resources | Connections to meal delivery, transportation, support groups, counseling services |
| Legal Referrals | Guidance on wills, estate planning, and referrals to legal professionals |
| Care Transitions | Assistance moving between home, hospital, nursing home, or assisted living |
| Funeral Planning | Support with end-of-life arrangements and paperwork after death |
Legacy Work and Life Review
Many patients want to leave something meaningful behind for their loved ones. Our social workers facilitate:
- Life review conversations that honor a person’s story
- Legacy projects like letters, videos, or memory books
- Conversations about values and life lessons to pass on
- Bucket list planning and fulfillment when possible
- Opportunities for reconciliation and saying goodbye
Bereavement Support
Our social workers continue supporting families after a patient passes away. This includes:
- Grief counseling for surviving family members
- Connection to bereavement support groups
- Resources for children grieving a parent or grandparent
- Guidance on normal grief reactions versus complicated grief
- Follow-up check-ins during the first year after loss
Supporting Chicago’s Diverse Communities
Chicago is home to approximately 2.7 million people from hundreds of cultural backgrounds. Our social workers respect and honor the diverse ways Chicago families approach death, dying, and grief. Whether your family is Irish Catholic from the South Side, Mexican-American from Pilsen, Polish from Jefferson Park, or part of Chicago’s growing Asian or African communities, we adapt our support to align with your cultural values and traditions.
We understand that different cultures have varying beliefs about:
- How openly to discuss death and dying
- Who should be involved in decision-making
- Expressions of grief and mourning practices
- Spiritual and religious rituals around death
- Family roles and responsibilities in caregiving
Our social workers work with your family’s values, not against them.
How Social Workers Fit into Your Care Team

Your social worker will:
- Visit your home regularly (frequency depends on needs)
- Conduct an initial assessment to understand your situation
- Be available by phone between visits
- Coordinate with other team members to ensure holistic care
- Adjust support as needs change over time
You control how much involvement you want. Some families need extensive support; others prefer occasional check-ins. We follow your lead.

The Difference a Social Worker Makes
Many families wonder if they really need a social worker. “We’re handling things fine,” they say. But hospice isn’t just about “handling things.” It’s about making the most of precious time, healing what can be healed, and finding meaning in the face of loss.
Social workers don’t fix everything, and they don’t take over. They walk alongside you, offering guidance when you need it and stepping back when you don’t. They help families have conversations they’ve been avoiding, make decisions that feel impossible, and find peace in the midst of pain.
Families who work with hospice social workers consistently say they couldn’t have managed without them. The practical help, emotional support, and steady presence make all the difference.
