Comfort-focused care can come to the place your loved one knows best, with practical support for the whole family.

Hospice is a coordinated form of care for a person with a terminal illness who has chosen comfort rather than treatment intended to cure that illness. The care plan is built with the patient, family, attending clinician when involved, and the hospice team.
At home, that plan may include:
Medicare explains that hospice services are based on the needs documented in the individual plan of care. That matters because two patients with the same diagnosis may need different visits, equipment, or support.
Care may take place in a private residence, assisted living community, or nursing facility. The hospice team works within that setting and coordinates with other caregivers when needed.
You do not have to wait for a crisis to ask questions. A family member, patient, physician, hospital, or care facility can contact Suncrest to start a hospice conversation.
If you are unsure whether it is time, call anyway. A conversation can help you understand the choices without committing to care.
Good home hospice care is steady and personal. The team should know the plan, explain changes in plain language, and make it clear who families can call with questions.
In this short video, Suncrest shares the care philosophy behind that approach. It is not a substitute for a clinical assessment, but it can help families understand what to expect from the relationship.
Yes. Hospice care is often provided in a private home. It can also be provided where a person lives, including an assisted living community or nursing facility, when the care plan supports it.
Hospice uses a team approach. Depending on the care plan, the team may include physicians, nurses, hospice aides, social workers, counselors, therapists, and trained volunteers.
Medicare Part A covers hospice care for people who meet its eligibility rules and elect the hospice benefit. Covered services must relate to the terminal illness and related conditions and be included in the plan of care.
Routine hospice care does not usually mean a staff member stays in the home around the clock. The team makes planned visits, teaches caregivers what to watch for, and explains how to reach the hospice when needs change.
Call Suncrest Hospice at 619-900-0335. A family member can start the conversation. The team will explain the next steps and coordinate with the patient’s medical providers.
